New Patient Form

At Milton Dentist, we electronically send your claims to your insurance on your behalf.  Payment is due at time of treatment, you will be refunded by your insurance provider normally within 24-48 hours.

For any changes to your scheduled appointment-respectfully the office requests 2 business days notice.
